Rent Well and its educational materials and curriculum are a registered trademark of TRANSITION PROJECTS, INC and therefore I understand that I cannot use any of the materials or curriculum in part or whole to recreate or reproduce without prior written consent.
I understand that while I may complete all the guidelines to graduate from the Rent Well tenant education course provided by the online eCourse managed by TRANSITION PROJECTS, INC, owner of the Rent Well Program that there are additional requirements to access the Rent Guarantee Program (RGP) beyond just my attendance and completion of the Rent Well eCourse.The Rent Guarantee Program managed by Oregon Housing and Community Services (OHCS) is paired with the Rent Well tenant education course and promises to pay the landlords of eligible Rent Well graduates up to $5000 if they leave their rental unit within the first 12 months and have any outstanding rental debts, damages, or eviction costs above and beyond what their security deposit can cover. In order to be eligible for (RGP), I need to provide extra documentation regarding that my monthly income amount is at or less than 60% area median income as defined by OHCS’s HOME/CBBG’s limits for the year I graduated from the course, as well as show documentation that I have at least one barrier to housing.
I give TRANSITION PROJECTS, INC, owner of the Rent Well Program permission to provide the documentation to verify my eligibility for the Rent Guarantee Program (RGP) to OHCS if need be for auditing and eligibility purposes. This verification includes, but is not limited to, the following:
- Verification of Income, including but not limited to pay stubs and government assistances letters
- Verification of Housing Status, including but not limited to, shelter entry/exit dates and reference letters
- Verification of Housing Barriers, including but not limited to, a copy of a background report, debt collection letters, civil reports, police records, rental ledger, credit report, reference letters, case management notes, and/or copy of housing portfolio/ cover letter
If I cannot provide documentation for any of the above, I understand that my self-certification on the registration form will be used instead. I hereby certify, under penalty of perjury, that I have not knowingly made, used, or caused to be made, a false record or statement material to a false or fraudulent claim and certify that this information, to determine my household eligibility for the RGP, is true, accurate and complete and that I do not have any documents or forms in my possession, nor am I able to obtain such documents to verify the statement made in this self-certification form. I understand this release is needed for the Rent Well Program to successfully register the RGP, on my behalf.
I give permission to review and verify my eligibility for the Rent Guarantee Program (RGP) with both Oregon Housing and Community Services, TRANSITION PROJECTS, INC, as well as with future landlords that I wish to use my Rent Well certificate with for up to 18 months after my graduation date.
Once the certificate and Rent Guarantee Program are registered with my future landlord, I give the Rent Well permission to speak to my landlord for up to 13 months beyond the move-in date of my new tenancy. This communication will be regarding if there is a need to use the RGP funding on my behalf if I move out in the first year of the tenancy and leave any unpaid rent, damages, or legal costs.
If this is not signed and access to the needed documents is denied, then the Landlord Incentive Fund cannot be registered, and I understand that I will be responsive for any and all debts related to my tenancy.
I am giving consent voluntarily and understand that I may, at any time, revoke it in writing to the entity giving or receiving the information. I have the right to see the information provided under this release at any time. My authorization releases the Rent Well Program, Transition Projects, Oregon Housing and Community Services, and my landlord/property manager from any and all liability for damages arising from inquiring about, obtaining, providing, and/or taking action based on information covered by this release. I have read this release, or it has been read to me, and I understand its content. I understand that I have a right to receive a copy of this release.
