Please fill out the Agreement form below, attach additional documents, and submit it within 30 days of the tenant’s occupancy. The packet must include:

Signed Copy of the Lease/Rental Agreement
Copy of the Move-In Condition Report/Inspection signed by both the landlord and tenant and
The Rent Guarantee Program Provider/Landlord Agreement signed (electronically or physically) by the landlord
